What is Greater Trochanteric Pain Syndrome?
Greater Trochanteric Pain Syndrome (GTPS) is a condition characterised by pain on the outer side of the hip, often caused by inflammation or irritation of the structures around the greater trochanter. These structures include the gluteal tendons and the trochanteric bursa (a fluid-filled sac that reduces friction between the tendons and the bone). GTPS is a common cause of lateral hip pain and can significantly impact mobility and daily activities.
GTPS is more prevalent in middle-aged and older adults, particularly women. However, it can also occur in younger individuals due to factors such as trauma, repetitive stress, muscle imbalances, and biomechanical issues.
What are the common symptoms?
- Pain and tenderness on the outer side of the hip may extend down the outer thigh.
- Discomfort when lying on the affected hip makes it difficult to sleep.
- Pain that worsens with activities such as walking, running, climbing stairs, or prolonged standing.
- Stiffness and weakness in the hip muscles lead to altered movement patterns.
- A feeling of snapping or clicking in the hip, in some cases.
How is GTPS diagnosed?
Your physiotherapist will assess your condition through a detailed history and physical examination, looking for the following:
- Tenderness over the greater trochanter.
- Pain when pressing on the affected area.
- Weakness or pain in the hip muscles, especially the gluteus Medius and Minimus.
- Movement or postural abnormalities contribute to the condition.
Reproduction of symptoms with specific tests, such as the Trendelenburg test or resisted hip abduction test.
Imaging tests, such as ultrasound, may be recommended to assess tendon integrity and rule out other conditions, such as hip osteoarthritis or referred pain from the lower back.
